Healthcare CostsWhat is a HC1 Claim Form ?The HC1 form can be used to claim help with NHS Prescriptions; NHS Dental Treatment; NHS wigs and fabric supports; sight tests, glasses and contact lenses; and travel to hospital for NHS treatment. The help is given on the basis of low income and most students on standard student income will qualify for help. How do I get a HC1 Form / check I can get help ?You can collect a HC1 claim form from a Social Security office or NHS hospital. Your doctor, dentist or optician may have one too or you can telephone for a form by contacting the Health Cost Advice line on 0845 850 1166. The form should be returned to: Patient Services, Sandyford House, Newcastle-upon-Tyne, NE2 1DB If eligible for a full exemption from charges the student will be sent a HC2 certificate. If eligible for partial exemption the student will be sent an HC3 certificate. The certificate are valid for 6 months or until one month after the start of the next academic year (whichever is longer) or, if in the final year, until the end of the course. You should claim in advance of becoming ill, otherwise you will need to use the form HC5 with an appropriate receipt to reclaim costs within 3 months of making full payment. Those who do not qualify but need several prescriptions would benefit by buying a "prepayment certificate" which gives unlimited prescriptions during its period of validity. The majority of University Undergraduate financial awards will not be counted if they are towards course related, as oppose to living, costs.
