Erasmus Grant Application Forms 2012/13
These are the grant application forms that students taking a Erasmus placement in the 2012/13 academic year must complete to apply for Erasmus funding. These forms must be completed and returned to the Erasmus Team at different stages of your placement. Please select the appropriate version of the form depending on whether you are undertaking a work placement or a study placement.

In total you will complete five forms one of which will be in two parts. The payment of your grant is dependent upon the return of all these forms.
Complete these before you leave
Please return forms 1 and 2 or 3 as soon as you can so that we have your details on record. Students whose details are not recorded by the deadline are likely to miss out on the Erasmus grant. Even if you do not start your placement until next year, you must still return these forms to Erasmus Team by the 30 September 2012.
If you are planning to participate in an EILC course, please contact us as this requires a different form.
1. Personal Details Form
2. Study Mobility Agreement or 3. Work Placement Mobility Agreement.
Prepare these before you leave
4. The Learning Agreement or 5. The Training Agreement
Form 4: The Learning Agreement (for Study Placements) and/or Form 5: Training Agreement (for Work Placements) attest that your placement is part of your degree and as such is a crucial part of your application. It is your responsibility to complete the form with advice from your Departmental Erasmus Advisor. You should get this signed by your Department and take the completed form with you when you leave for your placement. You will find your host university’s Erasmus ID on the attached document Erasmus ID Codes. You will find the NACE codes here (type of work placement sector).
Complete these during your placement
6. Certificate of Arrival and Departure – Part 1
Once you have arrived and registered at your host university/company you will need to get signatures on forms 4 or 5. Please return them as soon as you have obtained the signature. Without these forms, we will not be able to pay your grant to you.
Form 6 is in two parts. Part 1 (Certificate of Arrival) must be completed and signed by the host institution or company as soon as you arrive. We use this form to calculate the exact amount of money you will be paid for your grant, therefore it must be completed accurately.

6. Certificate of Arrival and Departure – Part 2
This must be completed at the end of your placement and confirms the dates given in Part 1. Should your placement duration as given in Part 2 be shorter than certified in Part 1, we reserve the right to reclaim the relevant part of your grant payment. You are only entitled to the correct amount as certified in Parts 1 and 2 of the Certificate of Arrival and Departure.
Student Report
You must complete a feedback report after your placement is completed. It will be conducted as an online survey and forms a compulsory part of your Erasmus Grant application. We will email the details of how to complete the online report form and publish them on the website as soon as they are available.
Payment Schedule
Erasmus grants are processed at different points (payment rolls) throughout the year. If you miss the deadline for one payment roll, your forms will not be processed until the next one.
In order to receive your payment as soon as possible please ensure that you complete and return all forms apart from the Student Report by the following dates.
- 31st October in order to be included in the November payment roll
- 31st January in order to be included in the February payment roll
- 30th April in order to be included in the May payment roll
- 30th June in order to be included in the July payment roll
